Student Organization Information Form

This form must be completed in its entirety and turned in 15 days after classes begin to the Director of Student Activities. If this form is not completed and returned by this time, your organization will not be allowed to attend allocations and your organization will be in jeopardy of not retaining SOC recognition for next semester.
