You will not be charged for testing and insurance is not required. If you have private health insurance, please bring that information with you.

Masks are required to be worn inside the testing site except when the tester asks you to remove your mask for swabbing.

Please bring your insurance information with you. Patients should not have any out-of-pocket costs, but you should check with your health plan to confirm scheduling a test. If you do not have insurance, bring your Social Security number (card not needed), driver’s license or state ID.

First Care accepts most major health insurance plans, Medicare, most Medicare Replacement and/or supplemental plans. We also accept most Fee-for-Service Medicaid and Managed Medicaid insurance plans.

To use your insurance, bring your primary insurance card, secondary insurance card (if you have one) and the policyholder’s name, date of birth and social security number.

For uninsured patients, we offer a Prompt Pay Program which includes an office visit, in-house testing, and procedures. If you have specific questions about your insurance provider, please call our clinic for clarification or visit our website.

Meridian Health Services has a COVID-19 virtual assessment visit with the NP who will then determine next steps for the patient with testing and treatment.
