Flu Vaccine Submission for Employees

Earlham is requiring all students and employees to get a flu shot this year.

To assist with securing your flu shot, Reid Health has generously donated vaccine doses to Earlham and we will be administering them in our Health Services Department.

If you are interested in receiving your vaccine through Earlham, you may visit the Health Services Office between 9 a.m. and 4 p.m. on Thursday, October 1 or Friday, October 2. These days are designated for faculty and staff and we have students being scheduled on other days.

Use of EC's Health Services is not mandatory. However, if you get your flu shot outside of Earlham, please submit your shot verification using the secure form below.

The deadline for getting your Flu Shot is Saturday, October 31.
