Equipment Request Form

Your faculty adviser must e-mail a letter to stating that she/he understands that they are responsible for the equipment when not in use. It must specify where the equipment is being stored, how it will be transitioned from convener to convener, and what will happen to it over the summer. This letter must be sent prior to the meeting with the SOC.
